Rural Primary Care Registered Nurse

We are expanding our team-based primary care clinic and are seeking RNs to jointly fill a 1.0 FTE position. This grant-funded role supports enhanced access to primary care through chronic disease management, panel optimization, and health education.

Medication administration, wound care, prenatal support

Position: Rural Primary Care Registered Nurse
Reports To: Clinic Manager (operational), NP Lead (Clinical Director)
FTE: 1.0 FTE total (may be shared among 2–3 staff)
Term July 2025 – June 2027 (grant-funded)
Clinic: Consort Medical Clinic
Schedule: Days with occasional after-hours &/or weekend coverage
Start Date: asap
Compensation: Competitive with UNA collective bargaining agreement
Purpose:
This role enhances access and continuity in rural primary care by supporting team-based, full-scope RN practice in partnership with NPs, and the team at the Consort Medical Clinic. Duties include patient triage and assessment, initial intake screening and history gathering, chronic disease management, preventative screening, health education, and clinic operations support.
Key Responsibilities:
A. Clinical Care and Preventive Services
Assess, plan, and implement nursing interventions within RN scope
Deliver health teaching and chronic disease management (e.g., diabetes, COPD)
Administer injections, immunizations, wound care, suture removal
Perform initial patient intake assessments, including prenatal assessment and teaching
Triage patients and/or perform standing order protocols for patients as designated by NPs
B. Panel & Population Health
Review EMR dashboards to identify and address care gaps
Organize patient recalls and preventive screening campaigns
Contribute to quality improvement, wait time reduction, and improved access
C. Team Collaboration
Participate in huddles and joint case management
Provide coverage across nursing staff (RN/LPN)
Assist with care coordination of same-day or walk-in clinic patients
D. Data, Documentation, and Evaluation
Maintain high-quality documentation in EMR
Support grant-related data collection (e.g., # patients served, operational hours, wait time improvements)
Cooperate with third-party program evaluators
Working Conditions:
Clinic-based work at Consort Medical Clinic, with potential for remote work for select tasks (e.g., patient follow-up calls, EMR care gap work, documentation)
Work-from-home eligibility based on:
Clinic space limitations
Nature and volume of work
Manager/Clinical Director approval and scheduling
All remote work must adhere to strict privacy and confidentiality protocols under the Health Information Act (HIA)
Interdisciplinary environment fostering autonomy, collaboration, and innovation
Qualifications:
Active registration with CRNA
Experience in primary care, community health, or chronic disease management an asset
EMR proficiency (Med Access preferred)
Strong interpersonal and independent judgment skills
