• Reports To: Clinic Manager (operational), NP Lead (Clinical Director)

• FTE: variable depending on scope and candidate

• Term May 2026 – April 2028 (grant-funded)

• Clinic: Consort Medical Clinic

• Schedule: Days with occasional after-hours &/or weekend coverage

• Start Date: asap

• Compensation: Competitive with collective bargaining agreements

This role enhances access and continuity in rural primary care by supporting team-based, full-scope RN practice in partnership with NPs, and the team at the Consort Medical Clinic.

As a Primary Care Nurse, you will:

• Provide patient-centered care across the lifespan

• Support chronic disease management, screening, and prevention

• Perform clinical assessments, triage, and in-clinic procedures

• Collaborate with Nurse Practitioners and the care team to implement care plans

• Provide patient education and health coaching

• Support care coordination and referrals

• Contribute to quality improvement and team-based care initiatives

• Maintain accurate documentation in the EMR (TELUS MedAccess)

• Clinic-based work at Consort Medical Clinic, with potential for remote work for select tasks (e.g., patient follow-up calls, EMR care gap work, documentation)

• Work-from-home eligibility based on:

  • Clinic space limitations

  • Nature and volume of work

  • Manager/Clinical Director approval and scheduling

• All remote work must adhere to strict privacy and confidentiality protocols under the Health Information Act (HIA)

• Interdisciplinary environment fostering autonomy, collaboration, and innovation

• Active registration with CRNA or CLPNA

• Experience in primary care, community health, or chronic disease management an asset

• EMR proficiency (Med Access preferred)

• Strong interpersonal and independent judgment skills
